Job Description
Visiting Nurse Association Visiting Nurse Association HouseCalls delivers primary medical care and consultation to persons who have difficulty getting to the doctor's office due to a chronic, progressive disease, or from sudden, escalating symptoms of an illness.
The Physician of VNA HouseCalls provides direct care of patients in the HouseCalls medical home program; acts as collaborating physician in support of VNA HouseCalls Advance Practice Nurses; oversees the medical Plan of Treatment for VNA HouseCalls patients; and assists in the administration of medical/clinical activities. The VNA HouseCalls Physician will act as an educational resource for both our staff and the greater community of professional and para-professionals regarding disease process and primary care management of essentially homebound patients.
IMMEDIATE MONDAY - FRIDAY 8:00AM -5:00PM OPPORTUNITY
FULL TIME, PART TIME & PRN
QUALIFICATIONS:
1. Licensed to practice medicine (MD or DO) in the State of Ohio . Minimum of five years clinical practice experience preferred
2. Experience as a physician in providing medical care (internal, geriatric, or family medicine) in community based settings preferably in the home environment. Knowledge regarding home health and hospice care.
3. Demonstrated knowledge and expertise in the medical management of elderly patient with chronic progressive illnesses; emphasis on pain and symptom control, psycho social management, and disease management.
4. Able and available to work as a member of the HouseCalls team. Experience and comfort working with Advance Practice Nurses in a collaborative practice arrangement required.
5. Experience with diverse populations.
RESPONSIBILITIES
1. In relation to VNA HouseCalls patients:
a. Examine and treat patients, consistent with standard of medicine practices of the State of Ohio .
b. Develop and maintain a primary care patient caseload.
c. Provide medical consultation/ home visits as requested by external primary care physicians.
2. In relation to VNA HouseCalls team members:
a. Provide collaborative physician support through a standard care agreement with designated Advance Practice Nurses.
b. Be available for and provide consultation with Advance Practice Nurses.
c. Provide continuing education to staff, where appropriate, and to other health providers as agreed.
